Municipality Of Kincardine Facilities Waivers (Member action required)

 Fellow Kinucks,  



As we get closer to our return to the ice there are a number of processes that need to be completed by all members.


Municipality of Kincardine requirements

·         The minor hockey waiver, accessed via the following link, needs to be completed for every player

·         Participant Waiver, accessed via the following link, needs to be read and signed by anyone over the age of 18 including participants, coaching  and team staff and any on ice helpers

These need to be completed by all prior to the first day KMHA returns to ice (04OCT)

 

There will be a number of contract tracing and health questionnaire requirements that will need to be completed each and every time you come to the rink as well. More details on what this process will look like are to follow shortly
