COVID Screening tool

     The Municipality of Kincardine requires that all ALL patrons entering the facility will need to complete an online screening questionnaire each time they enter the Davidson Centre and Tiverton Arena.

     Therefore, parents will have to complete screening via the below link for their child(ren)-screen shot the check mark, complete their own- screen shot the check mark and show Staff person at the door both screen shots upon entrance every time they come to the facility. To accommodate the extra steps, procedures have been revised to allow for arrival times 30 minutes prior to scheduled ice slots. When each parent, child, coach or volunteer have completed their screening and they have passed, a confirmation green check will appear that is time and date stamped. This needs to be done the day of facility visit. Please note this is in addition to the waivers which must be completed as well for players and volunteers at the start of the season.

     We are well aware that this has been a very frustrating process and unfortunately as we get going, we can foresee revisions still to come. On behalf of KMHA and the Davidson Centre Staff we thank you all for the patience and hard work you have put in to get our kids back to the ice.
