Lets make sure we do this right.

Fellow Kinucks,

     Please remember that ALL KMHA members MUST complete the following screening tool and waivers based on your level of participation. We have been advised that this may not be being done on a consistent basis and therefore poses a risk to our KMHA programming. Please check the below listed information and fill out any forms that have not been completed for either yourself or your player(s). 

The below screening form, accessed via the following link, is required to be completed by ALL PATRONS EACH TIME they enter the Davidson Centre and Tiverton Arena.


The below waiver, accessed via the following link, 
MUST be completed for EVERY player only once prior to taking to the ice

The below Waiver and Form, accessed via the following links, MUST be completed by EVERYONE over the age of 18 including participants, coaching and team staff and all on ice helpers only once prior to taking to the ice


The better we are the longer we can hope to keep the hockey season going. Let's do our best to stay healthy and stay active.
